Main duties of the job

The dedicated Healthcare Assistant is meant to work within the East Elmbridge Community Service alongside registered community nurses on twilight duty. The role involves providing care to housebound patients across Surrey Downs Health & Care localities. Your responsibilities will include delivering clinical interventions as delegated by senior team members, following personalised care plans, and providing personal, social, and emotional support to patients. You will also promote patient health and well-being, educate family members, maintain accurate records, and assist with administrative tasks. Strong teamwork, effective communication, safeguarding, and contributing to the Surrey Downs Health & Care Clinical Governance and Risk Management strategies are essential. Additionally, you will ensure a safe and efficient working environment.

Job description

Job responsibilities

Clinical

  • Primary responsibility will be to work with a registered community nurse on twilight duty for housebound patients in their own homes across Surrey Downs Health & Care localities you will also be required to undertake clinical interventions as delegated by a senior team member
  • Work as part of the team in the delivery of patient care, within the agreed guidelines
  • Treat all patients as individuals, respecting their privacy and dignity at all times
  • Follow individualised care plans devised by a senior team member and assist patients with personal, social, practical and emotional support as recorded

Health Promotion

  • To assist in the promotion of the health and well being of patients
  • Be involved in supporting, informing and educating family/carers where appropriate

Administration and Clerical Duties

  • Maintain accurate and clear record keeping
  • Participate in clerical duties as required by the team
  • Take and record telephone messages, respond and liaise appropriately
  • To be responsible for maintaining data entry and other record keeping

Team Working

  • Maintain regular contact with the senior team member and communicate all relevant issues regarding patient progress

General

  • Be able to work confidently at all competencies required for the role indentifying any areas of learning need to the senior team member as soon as they arise
  • Work in co-operation with all other professionals and agencies involved in the care of the patient
  • Be aware and act upon, where necessary, the procedures in place to protect vulnerable individuals
  • Participate in the Surrey Downs Health & Care Clinical Governance Strategy
  • Participate in the Surrey Downs Health & Care Risk Management Strategy
  • Undertake any such other duties as may be required from time to time as are consistent with the responsibilities and scale of this post
  • Maintain safe and efficient office environment e.g. tidy, hazard free
  • Have responsibility for the health, safety and welfare of self and others in the working environment and to follow safe working practice
  • Co owners are employed to work within Surrey Downs Health & Care localities and may be reasonably requested to move base temporarily or on a more permanent basis, as requested by service needs

Person Specification

Qualifications

Essential

  • Willingness to work towards NVQ Level 2
  • Willingness to learn new skills required for this post e.g. phlebotomy

Desirable

  • NVQ in Care Level 2 or equivalent qualification/course
  • Working towards an NVQ in Care Level 3 or equivalent

Experience

Essential

  • Previous experience of caring for patients

Desirable

  • Previous health care experience

Skills

Essential

  • Ability to work unsupervised and show initiative
  • Good interpersonal skills
  • Basic numeracy skills

Desirable

  • Clinical competencies as appropriate to the role
  • Computer skills

Other

  • Reliable
  • Car driver with clean UK licence
