Job summary
The Health Care Assistant (HCA) will work as an integral part of the multidisciplinary team across the Rural Warwickshire North Primary Care Network (PCN). Working under the supervision of healthcare professionals, the HCA will contribute to the delivery of high-quality patient care by performing clinical and administrative duties. This role supports patient health and wellbeing through routine observations, health promotion, and supporting practice teams in delivering essential primary care services.
Main duties of the job
As a key member of the clinical support team, the Health Care Assistant will contribute to a wide range of patient-facing and practice-based duties. Working under the direction of healthcare professionals, the postholder will support both the proactive and routine care of patients. This includes assisting in long-term condition management, helping to deliver national screening and prevention initiatives, and promoting positive health behaviours. The role also involves working closely with patients to ensure they are prepared for consultations, supporting clinical staff during procedures, and ensuring all necessary equipment and documentation are available and maintained. In addition to direct patient care, the HCA will be expected to uphold clinical governance standards, maintain confidentiality, contribute to a safe and organised clinical environment, and participate in wider service development activities across the PCN. This is a patient-focused role requiring compassion, attention to detail, and a strong sense of team collaboration.
About us
We are a Primary Care Network (PCN) of five practices working together with a range of local providers, including primary care, community services, social care, and the voluntary sector, to offer more personalized, coordinated health and social care to their local populations.
Our PCN comprises of 35k+ patients over the five practices, making us large enough to provide resilience and support the development of integrated teams.
The aims of a networks are to:
- Protect, improve, and extend local General Practice through greater organisational scale to provide convenient high quality, comprehensive and integrated services.
- Enhance the skills and flexibility of the General Practice workforce to meet population and service needs with programmes of continuing professional development.
- Work and collaborate with others to innovate in service provision, building on new technologies and recognising lifestyle choices of service users in the way in which they wish to access care.
- Ensure that all we do is valuable, both for the taxpayer and service user and for those investing their time and money into the PCN.
- Operate ethically, with accountability, probity, and fairness at the heart of the organisation.
