Job summary

The Health Care Assistant (HCA) will work as an integral part of the multidisciplinary team across the Rural Warwickshire North Primary Care Network (PCN). Working under the supervision of healthcare professionals, the HCA will contribute to the delivery of high-quality patient care by performing clinical and administrative duties. This role supports patient health and wellbeing through routine observations, health promotion, and supporting practice teams in delivering essential primary care services.

Main duties of the job

As a key member of the clinical support team, the Health Care Assistant will contribute to a wide range of patient-facing and practice-based duties. Working under the direction of healthcare professionals, the postholder will support both the proactive and routine care of patients. This includes assisting in long-term condition management, helping to deliver national screening and prevention initiatives, and promoting positive health behaviours. The role also involves working closely with patients to ensure they are prepared for consultations, supporting clinical staff during procedures, and ensuring all necessary equipment and documentation are available and maintained. In addition to direct patient care, the HCA will be expected to uphold clinical governance standards, maintain confidentiality, contribute to a safe and organised clinical environment, and participate in wider service development activities across the PCN. This is a patient-focused role requiring compassion, attention to detail, and a strong sense of team collaboration.

About us

We are a Primary Care Network (PCN) of five practices working together with a range of local providers, including primary care, community services, social care, and the voluntary sector, to offer more personalized, coordinated health and social care to their local populations.

Our PCN comprises of 35k+ patients over the five practices, making us large enough to provide resilience and support the development of integrated teams.

Protect, improve, and extend local General Practice through greater organisational scale to provide convenient high quality, comprehensive and integrated services.
Enhance the skills and flexibility of the General Practice workforce to meet population and service needs with programmes of continuing professional development.

Job description

Job responsibilities

Undertake routine clinical tasks such as:
Blood pressure monitoring
ECG recording
Phlebotomy (blood taking)
Weight, pulse, temperature, and BMI recording
Urinalysis and specimen handling
Support immunisation and vaccination programmes
Undertake NHS Health Checks as trained
Promote healthy lifestyles and provide basic health advice
Support with wound care, dressing changes, and assisting patients with mobility where necessary
Maintain accurate clinical records
Assist with stock control and equipment maintenance
Adhere to infection control procedures and health & safety policies
Maintain accurate, up-to-date clinical records on relevant systems (e.g., EMIS)
Ensure clinical areas are stocked, clean, and safe, following infection control protocols
Participate in team meetings, ongoing training, and development activities
Contribute to improving patient experience and supporting wider PCN service delivery

Person Specification

Qualifications

Essential

NVQ Level 2/3 in Health & Social Care or equivalent.
Experience working in a healthcare or care setting.
Good communication and interpersonal skills, ability to follow procedures, IT literate.
Understanding of patient confidentiality, infection control, and basic health promotion.
Compassionate, reliable, team player, flexible approach.

Desirable

Phlebotomy or ECG certification.
Primary care or general practice experience.
Experience with clinical systems such as EMIS.
Awareness of safeguarding and NHS Health Checks.
Willingness to undertake further training and development.
