About the Job:
Main duties of the job
To provide a supporting role to registered practitioners in the Integrated Community Team in the delivery of nursing care respecting patients’ privacy and dignity at all times.
To act as key worker for patients under the direction of the registered practitioners.
To assist in planning, delivering and evaluating programmes of care prescribed by the registered practitioners, regularly liaising at the beginning and throughout each shift.
To observe, monitor and record patient activity and health status as appropriate, communicating any changes in their condition to the registered practitioners.
To be responsible for progressing patient’s treatment plans within guidelines set by registered practitioners and regularly contributing to the review of each patient’s progress within the team
To perform a range of clinical nursing skills as directed by the registered practitioners:
Skin care/Pressure Ulcer prevention
Vital Signs – Temp/Pulse/Lying and Standing BP/Respiratory Rate
Mouth care/Eye care/Toe nails and Finger nails
Catheter care/Bladder washouts/Stoma care
Catheterisation (under registered practitioner discretion — NVQ III Training)
Continence re-assessment
Bowel care (suppositories)
Venepuncture
Basic Wound care
Simple bandaging and paste bandaging
To provide information and advice to patients and carers on how to optimise functional ability, nutrition, health and safety, and health promotion

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
