Job summary
We are seeking to appoint an enthusiastic, motivated and reliable Community Health Care Support Worker, who is keen to deliver a range of quality care services to patients visited by the Care Community Team based at Chowley Oak Tattenhall. The team visits patients of all the Rural Alliance Care Community Care Team. Being able to drive is an essential part of the roleAs part of a friendly team, you should have experience of working in patients’ homes, the ability to carry out tasks such as simple dressings, catheter care, personal care and recording blood sugars. This post involves some Lone working, providing care to those in the final days/weeks of life, supporting patients to prevent acute hospital admission and facilitating discharge from hospital.
The Trust currently supports the Real Living Wage (RLW) hourly rate as set by the Living Wage Federation and will apply a pay supplement if the national A4C hourly rate falls below this.
Main duties of the job
As part of a friendly team, you should have experience of working in patients’ homes and the ability to carry out tasks such as simple dressings, catheter care, personal care, recording blood sugars and have experience in end of life care. This post involves some Lone working providing care in patients homes. This post requires travel across the Western Cheshire footprint, access to transport for work purposes is essential.
