Job summary

An exciting opportunity has arisen within Hampshire and IOW Healthcare to appoint and enthusiastic Health Care Assistant (Band 3) to work within the multi-disciplinary Integrated Care Team that delivers community based services to a GP Practice population, undertaking nursing tasks within agreed parameters, set out by the registered practitioners.

The job will include home visiting and lone working without direct supervision.

The scope of the role will focus on supporting admission avoidance, early supported discharge and promotion of independence and assisting in the effective management of patients with long term conditions and older people with mental health problems.

We are looking for a motivated individual who is dynamic and keen to work as part of a team. The successful candidates will need to have relevant experience, good communication skills and the ability to work well as a team member. The Trust will provide training to complete the competencies required for this role. The Trust is also able to provide training opportunities to support ongoing career development.

Main duties of the job

  • To provide a supporting role to registered practitioners in the Integrated Community Team in the delivery of nursing care respecting patients’ privacy and dignity at all times.
  • To act as key worker for patients under the direction of the registered practitioners.
  • To assist in planning, delivering and evaluating programmes of care prescribed by the registered practitioners, regularly liaising at the beginning and throughout each shift.
  • To observe, monitor and record patient activity and health status as appropriate, communicating any changes in their condition to the registered practitioners.
  • To be responsible for progressing patient’s treatment plans within guidelines set by registered practitioners and regularly contributing to the review of each patient’s progress within the team.
  • To perform a range of clinical nursing skills as directed by the registered practitioners
