Main duties of the job
We are looking for compassionate and proactive Community Healthcare Assistants to join our dynamic community nursing teams. This role offers the unique opportunity to provide holistic care directly in patients’ homes, supporting their health and wellbeing within their familiar environment. Birmingham Community Healthcare NHS Foundation Trust provides a wide range of community-based health and social care services across Birmingham and the West Midlands. What we need from you:
- Minimum of 6 months experience in a community or home care setting is highly desirable.
- NVQ Level 2/3 in Health & Social Care or equivalent, and/or the Care Certificate.
- Strong organisational skills and ability to manage a caseload effectively.
- Proficient in using digital record-keeping systems.
- Ability to work autonomously and as part of a wider team.
- A commitment to continuous professional development.
- Delivering personal care, medication support, and basic wound care under the direction of community nurses.
- Monitoring vital signs, blood glucose levels, and escalating any changes in patient condition.
- Assisting with mobility, ensuring safe transfers and patient comfort.
