- Health Care Support Workers will assist in the provision of holistic health care, across Community Nursing, as delegated within their own level of agreed competency, to patients, families/carers in their own home setting.Ideal candidates will possess an NVQ Level 3 in Health and Social Care.
- As a member of our community multi-disciplinary team, you will work within a primary care network team alongside District Nurses, Specialist Nurses, Therapists and Health Care Assistant’s to meet the needs of complex community delivered patient care supporting early discharge and avoiding admissions to the acute sector. A full UK driving licence and access to a car for work purposes is essential.
